• End of Life Care and Hospice Support

Additional Services:

• Free Assessments of care available
• Local on- call staffing available 24 hours a day, 7 days a week
• Care Management Services offered by Director of Client Care personnel
• Primary care physician notifications provided
• Placement assistance for clients into LTC facilities
• Caregiver training specified to meet requirements of Individualized Care Plans for clients. (i.e. Rehab, Dementia, End of Life Care, Activities)
